Responsibilities
- Administer prescribed medications and treatments per physician orders
- Monitor vital signs, assess changes in patient status, and document findings
- Provide routine nursing care and assistance with activities of daily living; perform wound care and dressing changes as needed
- Collaborate with registered nurses and physicians; escalate changes promptly and appropriately
- Maintain accurate documentation in the PointClickCare EMR; ensure care plans are followed
- Float to all units within the same building as required; uphold safety and infection control standards

Requirements
- Active New York Licensed Practical Nurse (LPN) license
- Basic Life Support (BLS) certification
- Minimum 1 year of LPN experience in a long-term care or skilled nursing setting
- Ability to work evenings and float to multiple units within the building
- Proficiency with PointClickCare or ability to learn the EMR system
- Ability to meet facility safety and health requirements and pass background checks as required
