Compensation: $225,000 – $250,000 annually + leadership + high-impact clinical role .

Position Summary:

We are seeking a Physician (MD) to provide mobile primary care and rounding services across senior living communities. In this role, you will manage a panel of patients, deliver comprehensive care, and serve as a key clinical leader within each community. This is an opportunity to practice at the top of your license, influence outcomes, and build long-term relationships with patients and care teams in a growing, high-demand model.

Responsibilities:

• Provide on-site rounding and primary care services in assisted living, memory care, and long-term care settings.
• Perform diagnostics, treatment planning, and chronic disease management .
• Reduce ER visits and hospital readmissions through proactive, consistent care.
• Collaborate with advanced practice providers, nursing staff, and interdisciplinary teams .
• Act as a clinical leader and resource within each community.
• Communicate with families, caregivers, and facility leadership .
• Ensure accurate documentation, EMR utilization, and follow-up care .

Qualifications:

• Active Minnesota Physician (MD) license in good standing .
• Experience in geriatrics, internal medicine, family medicine, or post-acute care preferred .
• Strong ability to manage complex patient populations and make independent clinical decisions .
• Valid driver’s license and reliable transportation.
• Commitment to high-quality, patient-centered senior care .
