Your Impact:

• Patient Advocacy: Conduct in-home Medicaid/NYS assessments and reassessments.
• Clinical Accuracy: Ensure documentation and care plans meet all regulatory standards.
• Care Coordination: Collaborate with families and provide guidance to HHAs to ensure quality patient care.
• Expert Documentation: Maintain clear and timely records using our streamlined systems.

Requirements:

• License: Valid New York State RN License (Required).
• Transportation: Valid Driver’s License and reliable vehicle for field visits.
• Experience: Home Care or LHCSA experience strongly preferred.

Skills:

Strong clinical documentation habits and a patient-first mentality.
